| CAJUN Pet EXPRESS Shipping Procedures, Terms and Conditions In an effort to eliminate any misunderstanding, confusion or disagreements CPE has provided each customer with this copy of the Shipping Procedures, Terms and Conditions. Our goal is to provide a safe, friendly and affordable alternative to airline shipping and to represent our customers with the utmost professionalism and honesty. We value each and every customer and want to develop long term business relations that will serve both CPE and the customer. We love dogs and enjoy our profession. We also understand that your pet is very important and thus we treat each pet as if it was our own. We have not lost an animal period since we started shipping in 2005. Thank you for choosing CPE. 1. CPE offers a pet travel insurance policy that is available to each client or $500.00 per pet @ no charge. Please take advantage of this policy to cover your pet while in our care. In the event that your pet(s) should become ill or injured and should require veterinary care as the responsible party you authorize CPE to take the pet(s) to a licensed veterinarian for treatment. All bills incurred will be the responsibility of the owner of the pet(s) or insurance provider. I authorize CPE to make any emergency decision if they deem necessary for the well being of the pet(s) if I cannot be reached. I also agree to hold CPE harmless for any illness of my pet incurred during or after the transport of my pet unless it was the fault of CPE. 2. In order to provide a parasite free and clean environment for all pets while in transport we require that each dog is washed, dipped and or de-flea before pick up. Each pet will be inspected for fleas and ticks before boarding the transport van. CPE may charge an additional fee not to exceed $25.00 per pet for washing or dipping your pet if fleas or ticks are present. 3. Human aggressive dogs cannot be shipped by CPE. If an adult dog becomes human aggressive while in transport and poses a threat to CPE then I authorize CPE to bring the aggressive dog to a boarding facility or Veterinary Facility. CPE agrees to pay ½ of the shipping fee to cover boarding fees. The other ½ of the shipping fee will be non-refundable. The customer will then be responsible for additional boarding fees and the care, treatment and pick up of the aggressive dog. 4. A standard HEALTH CERTIFICATE issued by a Veterinary Doctor is required for puppies and adults. Medical records or shot records are not acceptable and cannot be used as a substitution. 5. PLEASE have this form, the health certificate and or copy of rabies certificate in an envelope upon pick-up of your pet. Please place name of receiver, city and state on the envelope. Any dog food given to CPE must be placed in a freezer bag and have the receivers name and city on it 6. All adult dogs shipped must be lead trained. CPE cannot ship a dog that resists hand walking. 7. CPE takes great pride in timely pick up and delivery of your pet. Our time is very important. Please be on time for pick ups and deliveries. Please have reliable transportation ready prior to meet our transport van. Also have a back up plan if your transportation fails. If you are going to be late for pick ups or deliveries please notify CPE. We have a one hour wait policy at which time an additional $25.00 fee may be accessed. CPE does not pick up and deliver pets after 11 PM or before 6 AM unless requested by the customer and agrees upon by CPE. 8. There is no additional charge for the administration of any medications unless otherwise stated. Any medications we are asked to administer must be dispensed specifically for your pet by a Licensed Veterinarian and must be sent in the container it was dispensed in reflecting your pets name, dosage and frequency. 9. All shipping fees must be paid to CPE upon pick up of your pet (s) to be delivered. Although it is not routine to accept COD payments, for whatever reason should this be necessary, I understand that if pick up of the pet (s) are not made or COD balance is not meet then all shipping fees will be required and additional fees will be required to ship the pet (s) back to the original pick up. The return transport price will be in accordance to distance and travel time but will not exceed the original shipping fee. All money due will be required prior to drop off. 10) CPE is a very safe transport service. We obey all speeding laws and apply for each state. We also implement defensive driving techniques. CPE to date has never received a speeding ticket or had been involved in an accident. We are aware that accidents do occur and in the event of an accident we will be solely responsible for making sure that your pet is well taken care. Also it is our responsibility to provide alternative transportation if out transport van breaks down. Our van gets regular service after each trip so that breakdowns do not occur. 11. CPE makes every effort to get your pet to its new home as quickly as possible without breaking speeding laws. We spend hours routing the trip so that the time spent on the road is kept to a minimal. The length of the transport of your pet(s) is determined by routing, weather and/or traffic and road construction. The national average for a ground transport is 3.5 days. Due to the traveling distance and route your transport could potentially last up to 5.5 days. While a 5.5 day transport is rare, it may occur, however, all efforts will be made to keep you informed as well as your pet(s) comfort throughout the transport. CPE will give you a date for pick up or delivery 2 days out. This allows you two days to plan make plans. We cannot drop off or pick up around your schedule. If you are working, on vacation or out of town during the assigned date of drop off or pick up then you must make arrangements to have someone else meet with us. 12. If a booking is cancelled 24 hours before the departure date listed on our web site then the $100.00 deposit is forfeited. Cancellations must be made no later than 24 hours before the scheduled departure before a refund is returned. 13. If the dog being boarded appears sickly we will not transport the animal and you will forfeit your $100.00 deposit. 14. CPE does not have additional room for your dog crates. We provide our own crates in the shipping van. This saves you money. You do not need to purchase shipping crates for your pet (s).
